Syniq Group LLC is seeking a qualified Case Manager to support state-funded programs. The ideal candidate will coordinate services, assess client needs, and ensure compliance with agency guidelines and documentation standards.
Conduct client assessments and develop service plans
Coordinate care and community resources
Maintain accurate case documentation
Ensure compliance with state and program regulations
Communicate effectively with clients and agency staff
Bachelor’s degree in Social Work, Human Services, or related field
2+ years of case management experience preferred
Strong documentation and organizational skills
Ability to pass required background checks
